LIABILITY WAIVER

Hokkaido Elite Club — Activity Participation Waiver

By completing this form, you confirm that you have read, understood, and agreed to the following terms before participating in any activity organised by Hokkaido Elite Club.

1. Voluntary Participation

I confirm that my participation in this activity is entirely voluntary. I understand that outdoor activities including fishing, stand-up paddleboarding (SUP), and snorkeling involve inherent risks including but not limited to: adverse weather conditions, physical injury, equipment failure, and unpredictable natural environments.

2. Assumption of Risk

I acknowledge and accept all risks associated with participation, including risks that may result from the negligence of Hokkaido Elite Club, its guides, or other participants.

3. Medical Fitness

I confirm that I am in adequate physical health to participate in this activity and that I have disclosed any medical conditions, allergies, or physical limitations that may affect my participation.

4. Minors

If I am booking on behalf of a minor (under 18), I confirm that I am their parent or legal guardian and accept full responsibility on their behalf.

5. Photography

I consent to photographs or videos taken during the activity being used by Hokkaido Elite Club for promotional purposes, unless I notify the guide otherwise on the day.

6. Cancellation & Refunds

I understand that cancellations made within 48 hours of the session are non-refundable. Weather cancellations initiated by Hokkaido Elite Club will result in a full refund or reschedule.
